  • Infant to 9 month Child Health councicling :

    Breastfeeding Beyond 6 Months :Internationally agreed recommendations for optimal feeding of infants and young children advocate exclusive breastfeeding for the first six months of life, followed by complementary feeding and continued breastfeeding for up to two years or beyond. From 6 months, infants need additional foods alongside continued breastfeeding. This is termed complementary feeding because the aim is to give other foods and drinks to ‘complement’, as in ‘make complete’, the nutrients provided by human milk. Appropriate CF is an advocacy issue because of lack of understanding and misinformation through the promotion of commercially produced follow-up and growing up milks.

  • 9 Month to Early Childhood Stage:

    Early childhood intervention is a support system for children with developmental disabilities or delays and their families.

  • School Age Children:

    School health services are services from medical, teaching and other professionals applied in or out of school to improve the health and well-being of children and in some cases whole families. These services have been developed in different ways around the globe but the fundamentals are constant: the early detection, correction, prevention or amelioration of disease, disability and abuse from which school aged children can suffer.

DIPLOMATE OF NATIONAL BOARD (DNB) IN PAEDIATRICS & CHILD HEALTH:

The Departmant of Pediatrics at Nirmal Hospital Private Limited was accredited by the National Board of Examination (NBE) to commence DNB (Pediatrics) course in 2011 after successfully establishing Fellowship programs in Pediatrics & Neonatal Intensive Care (2009). Nirmal Hospital is the only hospital in South Gujarat and the second only in the state of Gujarat to be accredited to run DNB Paediatrics program.

The National Board of Examinations was established in 1975 with the prime objective of improving the quality of the Medical Education by elevating the level and establishing standards of post graduate examinations in modern medicine on an all India basis. There are more than 149 recognized Medical Colleges in the country. In addition to Postgraduate teaching institutions under MCI, there are 450 accredited institutions imparting training in various Broad and Super specialities for the award of postgraduate qualification in Broad and super specialities. The Board at present conducts postgraduate and postdoctoral examinations in 54 disciplines approved by the Board for the award of Diplomate of National Board. The Medical Council of India has laid down standards for post graduate examinations conducted by various medical colleges and affiliated to concerned universities and other institutions, yet the levels of proficiency and standards of evaluation vary considerably in these institutions. The setting up of a National Body to conduct post graduate medical examination was intended to provide a common standard and mechanism of evaluation of minimum level of attainment of the objective for which post graduate courses were started in medical institutions. Moreover, intra country and international comparison is facilitated with the availability of commonly accepted evaluation mechanism.

RECOGNITION OF DNB QUALIFICATIONS:

The Nomenclature of the degree awarded by the National Board of Examinations is called the "Diplomate of National Board”. The list of recognised qualifications awarded by the Board in various Broad and Super specialities as approved by the Government of India and included in the First Schedule of IMC Act 1956 is as at Annexure II.

The Diplomate qualifications awarded by the National Board of Examinations have been equated with the postgraduate and post doctorate degrees awarded by other Indian Universities by the Government of India, Ministry of Health and Family Welfare; vide their notifications issued from time to time (Annexure III). The holders of Board’s qualification awarded after an examination (DNB Final) are eligible to be considered for specialists post in any Hospital including training/teaching institution.

  • The PICU of our hospital has been authorized to conduct one year certificate course in critical care by the academic council of the IAP-PIC.
  • The department is the ultimate referral centre for Paediatric care in the entire South Gujarat.
  • The department is recognized as Level III Intensive Care Unit by the national body.
  • Round the clock availability of Paediatricians is a unique feature of this department. In fact the Paediatric OPD at the hospital functions round the clock.
  • The department boasts of high frequency ventilator which is available with very few hospitals in the country.
  • Sophisticated life saving equipments like Multipara Monitors, Ventilators, Defibrillators, Pulse Oximeters etc help in delivering quality critical care.
  • Trained nursing staff to cater to the specific needs of Paediatric patients.

Nirmal Hospital Pvt. Ltd. has state-of-the-art, 7 bedded, level 3 Pediatric Intensive Care Unit Each bed has attached Multi-Para Monitors and facility to monitor Invasive Blood Pressure and Central Venous Pressure. The PICU is equipped with 4 conventional ventilators and shares two HFOV High frequency Oscillator Ventilators with NICU. PICU has approximately 800 admissions in a year with mortality rate of 8-10% which is at par with the National Statistics. In addition to the routine procedures Central Venous Line insertion, Arterial cathaterisation, ICD insertion, TPN administration, peritoneal dialysis and PICU can support Hemodialysis as well as Non Invasive Ventilation. PICU has also acquired Flexible Fiber Optic Bronchoscope and it will be used where indicated in managing children with respiratory problems. PICU staff actively supports the transport of sick children from the Surat city and from the towns with in radius of 100km outside Surat.

Paediatric Intensive Care Unit has two Full time Paediatric Intensivists Dr Jignesh Patel and Dr Jigesh Vaidya in addition to Dr Nirmal Choraria (Head of PICU & Chief Paediatrician) and Dr Rajiv Agrawal (Senior Paediatrician). FOLLOW-UP OF HIGH RISK NEONATES:

A significant proportion of babies who are discharged from NICU have certain high risk factors which means that they are at risk of growth or developmental delay. These babies need to be followed up closely so that any problems can be detected and treated at an earlier stage. These high risk factors include:

Nirmal Hospital Pvt. Ltd. is running weekly Growth and Development Clinic for these high risk neonates on every Wednesday between 12 PM and 2 PM. In this clinic, baby is examined at the ages of 3, 6, 9 and 12 months. For premature babies, corrected age is used (i.e. age is counted from the due date of delivery). The baby is examined by a neonatologist and a pediatric physiotherapist. Baby is checked for age appropriate developmental achievements and his/her growth is checked and plotted on a growth chart. At the end, parents are advised about baby's progress, areas where special attention is needed, baby's diet and further follow-up.

IAP-Neonatology Chapter recognized our institution in November 2008 for IAP-Neonatology Fellowship Program for two candidates every year commencing from January 2009. The Neonatology Chapter of the Indian Academy of Pediatrics is a subspecialty chapter of the Academy, which deals with academic, scientific and community activities conducted by the Academy for the care of newborn babies in the country. Beginning 2008, the Chapter started one-year Fellowship in Neonatology with the purpose of providing specialized training in Neonatology to students who have completed their post graduation in Pediatrics. The program will be conducted for candidates who are employed fulltime, as resident doctors, for a period of one year, in the institution, and the institution will conduct the training to be followed by Central examination.

Neonatal Intensive Care Unit is accredited by National Neonatology Forum of India. NNF has also accredited Neonatal Intensive Care Unit of Nirmal Hospital for NNF Neonatology fellowship program in 2013 for two fellowship candidates per year.

Nirmal Hospital Pvt. Ltd. has a modern, well-equipped and sophisticated neonatal intensive care unit (NICU), which can provide all three levels of neonatal care and has dedicated and experienced transport team to transfer newborn and children from other health facilities. The intensive care nursery has a capacity of 20 beds for level 2 and 3 care. There is a Paediatric Department which has 30,000 OPD attendance and 3,500 indoor admissions per year. NICU has the capacity to ventilate 9 babies at one time with invasive ventilation including 2 high frequency ventilators and monitoring with support of experienced medical and nursing staff. In addition, there is facility to provide non-invasive respiratory support with continuous positive airway pressure (CPAP) and bi-level positive airway pressure (BiPAP) devices. Bio Medical Engineer is available round the clock for the trouble free and smooth functioning of the life support equipment. NICU provides IV fluid administration with infusion pumps, hence accurate volume delivery is ensured. There are multiple multi channel vital sign monitors, which are capable of measuring blood pressure both invasively and non-invasively. Every baby requiring continuous pulse oximetry has its own dedicated pulse oximeter.

NICU has abilities to support Paediatric Surgeons and Cardiologists pre and post operatively. Cardiac Echo and EEG services are available from visiting sub specialists. There is availability of portable x-ray and ultrasonography round the clock and hospital has 24-hour laboratory support for urgent investigations including blood gas analysis. CT and MRI Scan are available in close by facilities and the baby is transported to and fro for these investigations accompanied by trained Paediatrician.

Neonatal Transport Services-The unit also provides 24 hrs neonatal transport services from Surat and South Gujarat by a paediatrician to retrieve a newborn and is able to provide artificial ventilation & IV fluid administration as and when required. Hospital plans to use transport ventilator to support transfer from outside Surat.

NICU STAFF:

The unit is staffed with four fulltime Consultant Neonatologists Dr. Nirmal Choraria, Dr. Rajiv Agrawal, Dr. Mohit Sahni and Dr. Jitesh Chavda. Parents have a chance to interact with one of them on a regular basis Dr Rajiv Agrawal and Dr Mohit Sahni have extensive working experience in Neonatal Intensive Care in Europe.

The IAP endorses the continued use of whole cell pertussis vaccine because of its proven efficacy and safety. Acellular pertussis may undoubtedly have fewer side-effect (like fever, local reaction at injection site and irritability), but this minor advantage does not justify the inordinate cost involved in the routine use of this vaccine.

If the mother is known to be HBsAg negative, HB vaccine can be given along with DTP at 6,10,14 weeks / months. If the mother’s HBsAG status is not known, it is advisable to start vaccination soon after the birth to prevent perinatal transmission of the disease. If the mother is HBsAG positive ( and especially HBeAg positive), the baby should be given Hepatitis B immune Globulin (HBIG) within 24 hours of birth, along with the HB vaccine.

Varicella, Hepatitis A and pneumococcal conjugate vaccine should be offered only after one to one discussion with parents. Also refer to the individual vaccines notes for recommendations.

Combination vaccines can be used to decrease the number of pricks being given to the baby and to decrease the number of clinics visit. The manufacturer's instruction should be followed strictly whenever “mixing” vaccines in the same syringe prior to injection.

At present the only, typhoid vaccine available is the vi polysaccharide vaccine. Revaccination may be carried out every 3-4 years.

  • Under special circumstances (eg.epidemics) measles vaccine may be given earlier than 9 months followed by MMR 12-15 months.

  • During pregnancy, the interval between the two doses of TT should be at least one month.

  • We should continue to use OPV till we achieve polio eradication in India. IPV can be used additionally for individual protection.

  • OPV must be given to children less than 5 years of age at the time of each supplementary immunisation activity.
